The Medical Foundation of Nashville’s Project Access Nashville Specialty Care supports health access by providing a coordinated system of specialty care for low-income, uninsured residents of Middle Tennessee.

Project Access Nashville Specialty Care (PANSC) accepts referrals from free and charitable primary care clinics, participating volunteer physician specialists, and the emergency departments of participating hospitals.

Once a referral is received, PANSC patients are connected with a care navigator who walk each patient through every step of the process, including:

  • Enrollment,
  • Appointment scheduling,
  • Interpretation service coordination, and
  • Community resource referrals,

The PANSC Care Navigator Team is fully bilingual in English and Spanish, and utilize interpreters for dozens of other languages spoken in our community.

Care navigators support PANSC patients and volunteer physician specialists as the patient and their physicians work together to diagnosis and create a treatment plan. PANSC care navigators work with patients to support attendance at healthcare appointments and adherence to healthcare instructions. PANSC patients have a 98% attendance rate, which helps volunteer physician specialists make the most of their time!

PANSC also works with community organizations to ensure patients have access to the resources they need including:

  • Transportation to healthcare appointments,
  • Address food insecurity through PAN Pantry,
  • Access to prescription assistance programs, and
  • Connections to assist other social determinants of health.

Thanks to recent expansion initiatives, PANSC currently serves patients from over 36 Middle Tennessee Counties, receiving referrals from 55 primary care clinics.

Through strong partnerships, PANSC has established a referral process based off a nationally recognized model that links patients to a network of community-based medical clinics, physician specialists, collaborating hospitals, and ancillary partners.

We are exceedingly grateful for the generous participation of our volunteer physicians and the following hospitals, which have donated the necessary inpatient and outpatient care to our Project Access Nashville Specialty Care enrollees, including surgeries, imaging and laboratory testing:

History: Since its 2005 inception, PANSC has coordinated over $66.7 million in donated medically necessary healthcare and over 46,687 patient appointments with physicians and hospitals to the uninsured, low income residents of greater Nashville. None of this would be possible without our network of over 2,100 volunteer physicians, hospital partners and community clinics.
